About The Position
The Center at Waterfront is a brand new facility that serves patients needing physical rehabilitation and/or complex nursing care. Our highly qualified, caring staff provides care in a beautiful, hotel like atmosphere to help our patients achieve strength and functionality. Skilled-nursing care, family involvement, psychological care and discharge planning complement a well-rounded, individualized program and encourages each patient’s transition to their highest functional level. Requirements
- Must meet the State educational requirements for a skilled nursing facility
- Must possess the ability to make independent decisions, follow instructions and accept constructive criticism
Nice To Haves
- Bachelors in Social Work
- 1 years’ experience in a skilled nursing facility and/or a long term care facility
Responsibilities
- Meets with administration, medical and nursing staff, as well as other related departments in planning services-to include patients and family
- Partners with the patient, family members, patient’s care team and social services staff, health and community agencies to insure a smooth transitions upon discharge from the Center
- Ensures that all documentation is informative, descriptive and follows the Center’s policy and procedures
- Assists with the admission process when needed
- Attends care plan meeting to provide insight and professional guidance to patients needs
