About The Position

Requirements

  • Bachelor’s degree in Social Work or related field from an accredited college or university.
  • Current Social Worker license in the State of Ohio.
  • Minimum of 2 years of experience in a healthcare setting preferred.
  • Strong problem-solving, organizational, and multitasking skills.
  • Excellent communication and customer service abilities.
  • Ability to maintain accurate and timely documentation.
  • Ability to work both independently and collaboratively as part of a team.
  • Compassionate, calm, and professional demeanor in emotionally demanding situations.
  • Ability to read, write, and speak English.

Nice To Haves

  • Minimum of 2 years of experience in a healthcare setting

Responsibilities

  • Provide social services support to residents, addressing psychosocial, emotional, and environmental needs.
  • Communicate with residents and families to resolve concerns, provide guidance, and ensure needs are met.
  • Complete and maintain comprehensive assessments, including social histories, care plans, and progress notes.
  • Advocate for residents’ rights, ensuring dignity, privacy, and respect at all times.
  • Collaborate with interdisciplinary teams, including staff, consultants, and healthcare professionals.
  • Maintain strict confidentiality of resident information in accordance with HIPAA regulations.
  • Support a calm, caring, and respectful environment for residents and staff.
  • Participate in care planning and contribute to individualized resident care strategies.
  • Attend required trainings, in-services, and meetings.
