About The Position

McMan Youth, Family and Community Services Association is a not-for-profit agency established in 1975, providing comprehensive programs across Alberta. They have built a strong reputation for being a responsive, competent, and innovative partner in service delivery, ensuring help is accessible to those in need. Their Coordinated Entry program in Red Deer is seeking a Permanent Full Time Coordinated Entry Outreach Facilitator to work with youths aged 15 to 24 who have experienced homelessness. The program accepts referrals for youth who are currently homeless, living on the street, in shelters, or in unsuitable accommodations, as well as those who are "couch surfing" or temporarily staying with others. The intake process involves an interview with the youth/family, reviewing available information, and discussions with referral sources and screening reports to prioritize cases.

Requirements

Diploma at minimum, preferred degree directly related to Human Services
1 year of frontline human services experience
Proof of Membership in Professional Association (when applicable)
Basic First Aid Level C with adult, child, and infant CPR &AED
Suicide Intervention Training & De-Escalation Training
Intervention Record & Criminal Record Checks with the vulnerable sector
Class 5 drivers license, clean drivers abstract and reliable vehicle for client transportation required.

Responsibilities

Develop relationships with homeless youth.
Work in partnership with Adult Coordinated Entry.
Support the warm transfer of youth from intake to housing.
Access appropriate community support for youth.
Work with external organizations to facilitate transitions and wraparound services.
Work to promote the program within the community.
