About The Position

Immanuel Pathways PACE® is seeking a Social Service Associate to work at 1702 North 16th Street, Council Bluffs, IA 51501. You will work with our Immanuel Pathways Day Center and at participant's personal homes. We reimburse $0.725/mile for travel. The schedule for this position is Monday through Friday 8:00 am to 4:30 pm, with no Weekends or Holidays. Pay will be based upon experience, certifications, skills and education.

Requirements

Applicants must be currently authorized to work in the United States on a full-time basis.

Nice To Haves

Bachelor’s degree in Social Work, Psychology or related field is preferred.
Two (2) years of experience working on a multidisciplinary team in a hospital, nursing home or community-based setting is preferred.

Responsibilities

Review of assessment completed by Master of Social Worker (MSW) and supports the treatment plan.
Teaching and educating of participant, caregiver or other appropriate representatives under the supervision of the MSW.
Social work interventions could include individual participant contacts; appropriate collateral contacts; participant and family education; provision of resources; ongoing case management; advocacy to ensure participant and caregiver needs are met and addressed; and disenrollment procedures under the supervision of the MSW.
The Social Services Coordinator may be the assigned liaison between the caregiver representatives and community agencies.
