About The Position
As a Social Services Assistant at Avalon Care Center Bellingham, you will have the opportunity to make a meaningful difference in the lives of residents and families. You will support residents through important transitions, advocate for their needs, and contribute to a positive resident experience from admission through discharge. This role is part of a compassionate, interdisciplinary care team within a CMS 5-Star Rated Facility.
Requirements
- Previous social services experience required
- Skilled nursing facility experience required
- Strong communication and interpersonal skills
- Ability to work effectively with residents, families, and interdisciplinary teams
- Strong organizational and documentation abilities
- Ability to pass required employment screenings
Responsibilities
- Support resident admissions, assessments, and discharge planning
- Assist residents and families with transitions and adjustment needs
- Advocate for resident rights and quality of life
- Participate in care planning and interdisciplinary team meetings
- Maintain accurate documentation and resident records
- Help connect residents and families with appropriate resources and support services
