About The Position

Vineyard Hills Health Center, a skilled nursing facility in Templeton, CA, is looking for that special person who has a strong desire to advocate for our residents. We are hiring for an Assistant looking to grow and be part of our team. Our Social Service Assistant helps residents with their basic needs by coordinating and acting as representatives for seniors who are unable to advocate for themselves. This is a full-time position. If you have a passion to care for seniors, this is the right opportunity for you! The Social Service Assistant provides administrative and client support to social workers and case managers. This role assists individuals and families in accessing social services and resources, maintaining accurate records, ensuring timely communication, and follow-up. The ideal candidate is compassionate, organized, and committed to helping vulnerable populations navigate support systems. Responsibilities include conducting admission interviews, informing residents and their families of their rights, advocating for residents with social services organizations, helping residents adjust to living inside and outside the facility, and coordinating care services.

Requirements

  • Organizational and recordkeeping ability
  • Basic understanding of social service systems and confidentiality laws
  • High School Diploma
  • Positive and nurturing personality, a true want to help and care for those in need
  • Open and flexible schedule, availability to work in the evenings or during weekends if needed
  • Proficient with Microsoft Office
  • Standing, lifting, twisting, bending, walking for long periods of time
  • Must be able to lift 50 pounds
  • Applicant must hold a valid drivers license

  • Experience in long-term, skilled nursing care a plus, but not required
