About The Position

Cadia Healthcare is seeking a Part Time Social Services Assistant to join their team. This role involves working with residents to identify and address their psychosocial, mental, and emotional needs, ensuring they have access to necessary services. The Social Services Assistant will be part of the Social Services department and will collaborate with residents and families.

Requirements

• Must possess a minimum of a Licensed Social Worker with a BSW
• Experience in a skilled nursing facility preferred

Responsibilities

• Review and implement policies regarding resident care and quality of life
• Assist in developing facility social work policies
• Develop and maintain a listing of current community resources that will be utilized by the residents and their families
• Document progress in meeting the psychosocial needs of residents to include thorough and informative completion of all required forms and reports, which are to be submitted to the Administrator
• Ensure that Social Services MDS reporting requirements are completed properly and in a timely manner to meet regulatory requirements
