About The Position

Assists with daily living activities, vocational training, community integration, and service plan implementation in community-based and residential behavioral health settings. Provides instruction, supervision, life skills training, employment readiness support, transportation, and recovery-oriented services to individuals receiving behavioral health services. Works closely with treatment team members to assist individuals in achieving individualized goals and improving functioning, independence, and quality of life.

Requirements

  • Knowledge of agency confidentiality policies and HIPAA requirements.

  • Knowledge of behavioral health service delivery, documentation requirements, and treatment planning.
  • Knowledge of state, federal, and local regulations governing behavioral health services.

  • Ability to observe and monitor participant behavior and progress.
  • Ability to assist and supervise participants with daily living and community integration activities.

  • One (1) year of experience providing social or direct care services to individuals with special needs in a human services setting; OR Associate degree in a related field; OR One (1) year of experience at the lower-level position or equivalent.
  • Valid Georgia Driver’s License required.
  • Upon appointment to the position, the employee assumes responsibility for meeting and maintaining all minimum qualification requirements, including applicable certifications, licenses, continuing education, competency validations, and mandatory annual or recurrent training.

Responsibilities

  • Ensures consumers receive services that address identified needs, goals, and objectives.
  • Meets established productivity expectations for direct service delivery.
  • Provides instruction and support regarding daily living, social, community, and vocational skills.
  • Observes and monitors consumer behavior and progress.
  • Provides guidance regarding daily decision-making and problem-solving.
  • Assists in development and implementation of Individual Service Plans and treatment plans.
  • Participates in treatment team meetings and case reviews.
  • Provides support, coaching, and skill-building activities designed to enhance independence and recovery.
  • Provides prevocational and employment readiness training in designated work settings.
  • Supervises and trains consumers participating in Community Access Group activities and work-related services.
  • Ensures vocational activities support consumer ISP goals and objectives.
  • Tracks and maintains accurate records of vocational activities, productivity, and outcomes.
  • Ensures consumer work time and compensation records are accurate and complete.
  • Promotes development of work-related skills, responsibility, and community participation.
  • Operates agency vehicles to transport consumers to and from programs, appointments, community activities, and outings.
  • Provides transportation for Community Access activities and other approved services.
  • Ensures compliance with Department of Motor Vehicles regulations and agency transportation policies.
  • Conducts required vehicle inspections and ensures assigned vehicles are maintained and cleaned appropriately.
  • Promotes safe community integration and participation.
  • Prepares and maintains records of consumer progress and services provided.
  • Ensures documentation accurately reflects goals, objectives, and consumer outcomes.
  • Completes required reviews, annual documentation, and service records within required timelines.
  • Documents services within 24 hours of service delivery.
  • Ensures documentation supports services billed and complies with agency, payer, and regulatory standards.
  • Provides supervisors with documentation when required for review and billing purposes.
  • Maintains accurate records and reports significant changes in consumer condition or functioning.
