About The Position

Assess and support home-delivered meal clients (existing and newly referred) under the Title III Older American’s Act. The work in this position takes place primarily on the phone or in the client’s home. This role operates in a professional office environment and routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines. The work environment can be stressful at times, with moderate noise levels and the need to work in a confined area. The role requires regular, predictable attendance.

Requirements

  • Bachelor’s degree in social work or related field.
  • Excellent verbal, written, and electronic communication skills.
  • Demonstrated proficiency in use of computer programs (Microsoft Outlook and Office).
  • Demonstrated proficiency in database management.
  • Strong planning and organization skills.
  • Current and valid Ohio driver’s license and automobile insurance or reliable transportation services for home-based assessments.
  • Attention to detail and accuracy.
  • Ability to prioritize work and consistently meet deadlines.
  • Ability to work as a team player with all departments.
  • Minimum 3 years’ experience in social work, counseling, or related field.

Responsibilities

  • Performs assessments to determine initial and ongoing eligibility for Title III home delivered meals.
  • Coordinates appropriate referrals and resources for clients through phone or in-home visits, such as benefits counseling (medical insurance claims, social security, SSI, Food Stamps, Medicaid), in-home assistance, and transportation.
  • Maintains an active and comprehensive understanding of the spectrum of Home and Community Based Services (HCBS) that Benjamin Rose has to offer, as well as older adult services, programs, and resources available in the community.
  • Performs necessary data entry into client management software databases for reporting of services provided and reimbursement purposes.
  • Participates, as needed, in ongoing case conferences with home delivered meals staff, other Benjamin Rose program staff, affiliated agencies, and clients and caregivers.
  • Provides information and referral to Benjamin Rose staff and other community agency personnel.
  • Manages external referral platforms.
  • Responsible for all documentation related to clients received services.
  • Works closely with home delivered meals staff to ensure needs of active and future clients are being met.
  • Develops and maintains a network with other community resources.
  • Other duties as assigned.
