About The Position

The Social Services Director at Pacific Coast Post Acute leads the delivery of care to residents and families in Salinas, CA. This role ensures every individual reaches their peak physical, emotional, and psychosocial well-being. As a PACS-supported facility, we provide a nurturing environment that bridges the gap between hospital and home with personalized, person-centered services.

Requirements

• Bachelor’s in Social Work or Human Services required
• 2 years of supervised social work in a healthcare setting.
• Ability to interpret complex instructions and solve practical problems in a fast-paced environment.
• Frequent standing, walking, reaching, and pushing/pulling.
• Regular lifting of up to 25 pounds.
• Frequent communication (talking/hearing) and sensory awareness.

Nice To Haves

• Master’s (MSW) preferred.
• Experience with PointClickCare (PCC) is highly preferred.

Responsibilities

• Partner with residents and families to develop individualized care plans and set reachable goals.
• Deliver medically related social services and evaluate psychological needs to sustain mental health.
• Provide counseling and support for residents coping with health anxieties, grief, or physical transitions.
• Connect residents with community welfare agencies and secure necessary assistive technology.
• Manage personal shopping, track belongings, and schedule on-site services like dental or psychiatric care.
• Lead discharge planning and referrals to ensure a safe, successful return home.
• Supervise staff, conduct in-service training, and foster strong relationships with internal departments.
