About The Position
The Social Service Director is responsible for guiding residents and their families through the care process, assessing cognitive capacity and mood, implementing healthcare directives, and understanding end-of-life care and hospice. This role also involves discharge planning and collaboration with the healthcare team, including nursing and physicians.
Requirements
- Minimum 2-3 years’ experience in long term care and skilled nursing care
- Bachelors in social work
- Demonstrate strong conflict resolution
- Therapeutic relationship skills
- Insurance/Medicare proficiency
Responsibilities
- Appropriately transition the resident and family through the care process
- Access cognitive capacity and mood assessments utilizing standard tools
- Implement healthcare directives and ability to have discussions and knowledge of end of life care and hospice
- Understand fundamentals of discharge planning and able to work collaboratively with healthcare team which includes nursing and physician
