About The Position

This position serves as the first point of contact with patients, responsible for intake and referral of services to clients applying for admission to the clinic and for clients being released from the hospital. The role involves documenting screening processes and collaborating with other clinic staff to schedule appointments and refer clients to appropriate resources. The Social Service Counselor will focus on understanding and meeting customer needs, making accurate judgments, thinking critically, accepting direction, building trust, developing plans, following policies and procedures, managing time effectively, and using data to inform decisions. Requirements

Three years of social services experience; OR Six years of full-time experience in any field; OR A Bachelor's degree.
Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.
Positions that provide addictive disorder counseling or prevention services will require certification or eligibility for certification issued by the Addictive Disorder Regulatory Authority (ADRA), or its successor.

Responsibilities

Coordinates the referral process of all clients requesting services and routes them to appropriate services.
Takes phone calls and handles non-emergency walk-in clients.
Schedules appointments for high-risk and priority populations.
Conducts brief clinical assessments to determine the need for admission for mental health and/or substance abuse services.
Assesses the urgency of need for clinic services and schedules appointments accordingly.
Completes necessary documentation of registration forms and enters client information in Carelogic.
Communicates with referring hospitals and/or community agencies to ensure appropriate information is obtained and to address case-specific problems with continuity of care.
Makes clinical assessments regarding the appropriateness of referrals and makes alternative referrals as necessary.
Serves as a back-up crisis worker.
Addresses specific problems with access to care with the supervisor and/or clinical manager.
