About The Position

Responsible for providing support and care to meet the physical and psychological needs of consumers through providing direct and indirect care. BHUC is a 24-hour behavioral health urgent care service.

Requirements

• Tech I: Para Professional services are delivered by staff who have the life experience of being diagnosed with a serious mental illness or substance addiction problem, and are currently in recovery, stable, and living without relapses.
• The Paraprofessional has a high school diploma, or GED and at least five years of life experience with a serious mental illness or substance addiction.
• Peer Support - Requires Peer Support Certification.
• AP - requires high school diploma and at least one year of appropriate experience.
• Or Bachelor’s degree in the field with no experience.
• Tech II: QP- requires bachelor's degree in social work, Counseling or another Human Services field.
• Qualified Professional (QP) required.

• Minimal supervision regarding use of time, able to prioritize work assignments.
• Ability to communicate effectively with professionals and clients/families.
• Ability to make sound decisions in emergency situations.
• A desire to work with individuals with severe mental illness and substance abuse issues.

Responsibilities

• Work with referral sources and community partners to identify applicants through team meetings, outreach, and referrals.
• Complete interviews with individuals to gather information to complete SSI/SSDI applications
• Gather medical records and other information to complete SSI/SSDI applications
• Write SOAR Medical Summary Reports for individual applications
• Accompany individuals to appointments at the Social Security Administration
• Coordinate visits to medical doctors, psychiatrists, and other specialists to obtain evidence for the application
• Coordinate case management services with partners and help with providing case management services to individuals when needed.
• Assist the team with administrative tasks as needed.
