This position provides services to assigned children, youth, and families in various community-based settings. The role involves Intensive Home Based Services (IHBS) and/or individual rehabilitation, collateral support to caregivers, and the development and facilitation of skill-building groups. The Skills Trainer is responsible for completing assessments, crisis plans, and various documentation packets, while also collaborating with agency and community partners. The position requires participation in team meetings, on-call rotations, and ensuring safe transportation for clients. Additionally, the role involves working collaboratively with internal programs and staff, maintaining positive relationships with consumers, and meeting documented outcomes.
