About The Position

Assesses the ability to independently coordinate tasks associated with the Personal Care Management (PCM) program, assists consumers in developing skills and resources to manage PCA services and activities of daily living.

Requirements

• Written and Verbal Fluency in English and Spanish is required
• High School Diploma or equivalent is required
• Valid driver's license
• Reliable transportation
• Must be able to climb multiple flights of stairs
• Good assessment skills
• Knowledge of community services
• Good oral/written communication
• Data entry skills

Nice To Haves

• Bachelor's degree is preferred
• Hybrid schedule may be possible after six months

Responsibilities

• Conducts home visits
