Requirements

  • Active Maryland RN License (DC and/or VA a plus)
  • 2+ years of nursing experience
  • Experience supervising, mentoring, or leading clinical staff
  • Strong assessment, care planning, and clinical decision-making skills
  • Valid driver's license and reliable transportation
  • Excellent communication, organizational, and documentation abilities

Nice To Haves

  • Home Health or Skilled Home Care experience
  • OASIS experience
  • Multi-state licensure (MD, DC, VA)
  • Experience with AlayaCare or other EMR systems
  • Associate Degree in Nursing or higher

Responsibilities

  • Champion Exceptional Patient Care: Conduct admissions, assessments, recertifications, and change-in-condition visits. Develop and oversee individualized care plans. Monitor patient progress and intervene proactively when clinical needs change. Collaborate with physicians, caregivers, and interdisciplinary teams to achieve optimal outcomes.
  • Lead and Develop Clinical Teams: Mentor, coach, and support field RNs and LPNs. Conduct supervisory visits, chart audits, and competency evaluations. Provide real-time clinical guidance and problem-solving support. Foster a culture of accountability, excellence, and professional growth.
  • Drive Quality and Compliance: Ensure adherence to agency policies, regulatory standards, and clinical best practices. Maintain accurate and timely documentation within the EMR platform. Support quality initiatives, audits, and accreditation readiness. Uphold the highest standards of patient privacy and HIPAA compliance.
  • Help Grow Our Skilled Services Program: Partner with hospitals, physicians, discharge planners, and referral sources. Support smooth patient onboarding and continuity of care. Serve as a trusted clinical ambassador in the community.
