About The Position
LPN needed for skilled nursing facility. Days and Nights available (12 hour shifts).
Requirements
- Active Licensed Practical Nurse (LPN) license in good standing
- Graduate of an accredited practical nursing program
- Current CPR/BLS certification
- Strong clinical assessment and communication skills
- Ability to work in a fast-paced environment
- Physical within the last year
- PPD within the last year
- MMR shot records
- Hep B shot records (can be declined)

- Experience in skilled nursing, long-term care, or rehabilitation preferred

- Attention to detail and strong documentation skills
- Ability to prioritize tasks effectively
- Team-oriented mindset
- Knowledge of electronic medical records (EMR) systems
Responsibilities
- Administer medications and treatments as prescribed by physicians
- Monitor and record vital signs, symptoms, and changes in patient condition
- Assist with wound care, IV therapy (as permitted by state regulations), and other skilled procedures
- Provide support with activities of daily living (ADLs), including bathing, dressing, and mobility
- Collaborate with interdisciplinary team members to develop and update care plans
- Document patient care accurately and in a timely manner
- Educate patients and families about health conditions and post-care instructions
- Ensure compliance with facility policies, state regulations, and infection control standards
