About The Position
Join Senior Doc at Brigade Health in redefining care for medically complex and aging populations within Skilled Nursing Facilities. We’re seeking a compassionate, clinically strong Physician (MD/DO) to serve as a key medical leader within our SNF partnerships — combining hands-on patient care with clinical oversight and interdisciplinary collaboration.
Requirements
- MD or DO with active unrestricted medical license
- Board Certified/Eligible in Internal Medicine, Family Medicine, Geriatrics, or related specialty
Nice To Haves
- Experience in SNF, post-acute, geriatrics, or long-term care preferred
- Passion for relationship-centered, patient-focused care
Responsibilities
- Lead and support high-quality care for SNF residents
- Partner with facility leadership, nursing, and care teams
- Manage complex geriatric and post-acute cases
- Drive quality outcomes, compliance, and care coordination
- Support transitions of care, goals-of-care discussions, and operational excellence
