About The Position

Woodridge Rehabilitation & Nursing at Central Vermont Medical Center (CVMC) is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our interdisciplinary team in providing high-quality care to a complex adult and geriatric population. In this role, you’ll care for both long-term residents and post-acute patients recently discharged from the hospital, offering a unique blend of continuity and acuity within a collaborative, team-based environment.

Requirements

  • NP or PA with (or eligible for) Vermont licensure
  • BLS certification required
  • Comfort managing medically complex adult or geriatric populations
  • Ability to work independently while collaborating closely with a multidisciplinary team
  • Nurse Practitioner (NP) Graduate of a Nurse Practitioner program (Master’s or Doctoral degree) approved by the Vermont Board of Nursing or national accrediting agency
  • Maintains active licensure as a registered nurse (RN) and active, unrestricted licensure to practice as an Advanced Practice RN (APRN) by the Vermont Board of Nursing
  • Physician Assistant (PA) Successful completion of a Physician Assistant program at an institution accredited by ARC‑PA (Accreditation and Review Commission on Education for the PA)
  • Current certification by the National Commission on Certification of Physician Assistants (NCCPA)
  • Active, unrestricted licensure to practice as a PA by the Vermont Board of Medical Practice
  • DEA registration (or eligibility)

Nice To Haves

  • 2+ years of experience in primary care, hospital medicine, urgent care, or post-acute care
  • RNFA certification strongly recommended
  • Physician Assistant (PA) Bachelor’s degree required; Master’s degree preferred
  • Given the complexity of the patient population, candidates with prior APP clinical experience are strongly preferred

Responsibilities

  • Provide comprehensive inpatient care in a skilled nursing and rehabilitation setting
  • Manage a panel of approximately 6–8 patients per day
  • Assess and manage acute changes, chronic conditions, and post-hospital transitions
  • Lead daily rounding, including review and triage of overnight events with nursing staff
  • Support safe and efficient discharges back to primary care providers
  • Collaborate closely with physicians, nurses, rehab therapists, and care managers
