Requirements
- Active Registered Nurse (RN) license in the state of employment
- Graduate of an accredited School of Nursing
- Valid driver’s license and reliable transportation
- Current CPR certification

- Previous Home Health experience
- Experience with OASIS documentation
- Strong assessment and critical thinking skills
Responsibilities
- Perform comprehensive patient assessments including OASIS assessments when required
- Develop, implement, and update the Plan of Care in collaboration with physicians and the interdisciplinary team
- Provide skilled nursing treatments and interventions as ordered by the physician
- Monitor patient progress and document outcomes of care
- Communicate patient status and changes to physicians and the care team
- Coordinate services with therapists, aides, and other healthcare professionals
- Ensure timely follow-up on physician orders, lab results, and care updates
- Maintain accurate and timely clinical documentation
- Ensure compliance with CMS, Medicare, CHAP, and agency standards
- Participate in clinical reviews, quality assurance, and chart audits when requested
- Educate patients and caregivers on disease management, medications, and safety
- Provide guidance on preventative care and recovery strategies
- Supervise LPN/LVN staff and home health aides when appropriate
- Participate in meetings, training programs, and agency in-services
- Support quality improvement initiatives and best practices in home health care
