Requirements

Active Ohio Licensed Practical Nurse (LPN) license in good standing
Graduate of an accredited practical nursing program
Current BLS/CPR certification
Valid driver's license, reliable transportation, and current auto insurance
Strong communication, organizational, and documentation skills
Ability to work independently in a home health environment

Nice To Haves

Previous home health or hospice experience
Experience with Homecare Homebase (HCHB) or another EMR
Wound care or IV therapy experience
Bilingual candidates are encouraged to apply

Responsibilities

Provide skilled nursing care in patients' homes
Assess and monitor patient conditions
Administer medications and treatments as ordered
Perform wound care and other skilled nursing procedures
Educate patients and caregivers on medications, disease processes, and care plans
Document all patient visits accurately and timely using HCHB
Communicate changes in patient condition to the RN Case Manager and physician
Collaborate with the interdisciplinary care team to ensure excellent patient outcomes
Maintain compliance with agency, state, and federal regulations
