About The Position

This position is responsible for ensuring the RAI (Resident Assessment Instrument) is completed and electronically transmitted within federally mandated time frames. The Skilled Care Coordinator will develop a written plan of care for each patient according to federal regulation. This role assists leadership in planning, developing, organizing, implementing, evaluating, and directing the Nursing Service Department, as well as its programs and activities, in accordance with current rules, regulations, and guidelines that govern the long-term care facility. This position carries out the policies and procedures established by the facility and participates in facility inspections made by authorized government agencies.

Requirements

Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).
Two (2) years long-term care experience

Responsibilities

Assist leadership in planning, developing, organizing, implementing, evaluating and directing the Nursing Service Department as well as its programs and activities in accordance with current rules, regulations and guidelines that govern the long-term care facility.
Participate in facility inspections made by authorized government agencies.
In the absence of leadership, performs the duty of carrying out the policies and procedures established by this facility, and coordinating other duties as directed by NHA.
Rotates On Call with other Nurse Management level associates.
Works closely with DON to assure effective utilization and review.
Deals tactfully with personal, residents, family members, visitors, government agencies/personal and the general public.
Encourage attending physician to record and sign physician’s orders, certification form, DNAR form and Psychotropic drug form on a timely basis and in accordance with current regulations.
Ensure the RAI is completed, verify that the MDS is correct before locking it and electronically transmitted within federally mandated time frames.
Monitor the MDS and implement the CAA’s per policy and procedure.
Ensure each portion of the assessment is signed and dated by the person completing that portion of the MDS per policy and procedure.
Sign and date the assessment instrument to certify its completion per policy and procedure.
Review discharge charts and ensure the nursing portions are complete.
Accurately generate, EDS file, receives validation and confirmation and maintain records thereof.
Participate in the development of preliminary and comprehensive assessments of the nursing needs of each patient per policy and procedure.
