About The Position

Provides Mental Health Skill Building Services to individuals with serious mental illness to support community stability, independence, and successful functioning in the least restrictive environment. Provides individualized training, coaching, support, and transportation to help individuals develop and maintain skills related to health and safety, personal care, activities of daily living, medication management, community integration, and use of community resources. Services are provided in accordance with each individual's service plan, applicable regulatory requirements, and agency policies.

Requirements

Bachelor's degree in a Human Services field.
QMHP certification or eligibility to obtain QMHP-T certification.
Valid driver's license.
Knowledge of serious mental illness, recovery-oriented services, and person-centered practices.
Knowledge of Mental Health Skill Building Services, rehabilitative interventions, and teaching, coaching, and skill-development strategies.
Knowledge of crisis prevention, de-escalation, behavior-support strategies, and appropriate mental health interventions.
Knowledge of community resources, benefits, transportation, housing, and other supports that promote community stability and independence.
Knowledge of individual rights, confidentiality, professional boundaries, and applicable documentation and regulatory requirements.
Ability to establish effective professional relationships while maintaining appropriate boundaries.
Ability to work independently in office, home, and community settings and maintain a flexible schedule based on service needs.
Ability to protect confidential information and complete required records accurately within established timeframes.
Clear oral and written communication, accurate documentation, organization, and time management skills.

Nice To Haves

Minimum one year of experience providing behavioral health services to individuals with mental health, intellectual disabilities, and/or substance use issues.

Responsibilities

Provide Mental Health Skill Building Services to individuals with serious mental illness.
Provide individualized training, coaching, support, and transportation to build and maintain skills related to health and safety, personal care, activities of daily living, medication management, community integration, and use of community resources.
Assess individuals' strengths, needs, progress, and barriers to independent living.
Teach and reinforce daily living, health and safety, medication-management, social, and community-integration skills.
Coordinate services with individuals served, families, agency staff, providers, and community partners.
Plan and coordinate activities that support each individual's service plan and identified goals.
Complete required records accurately within established timeframes.
