About The Position

Meridian Health Services is a progressive healthcare organization that believes in treating the "whole person" by integrating physical, mental, and social well-being. They are currently seeking a full-time, day Skill Builder for their Children's Program in Tippecanoe County, IN. This role involves providing skills and development training to assist patients and/or families in achieving developmental milestones and reducing psychosocial barriers. The Skill Builder will also provide case management services and work collaboratively with the clinical team for care coordination, implementing the team-determined treatment plan on a day-to-day basis.

Requirements

  • High school diploma or its equivalent with 2 years paid behavioral healthcare experience or equivalent college coursework toward a human services or health-related degree required at minimum.
  • A valid driver's license with a driving record meeting Meridian's driving policy.
  • Reliable transportation.
  • Proof of auto insurance.

  • Associate or bachelor's degree preferred.

Responsibilities

  • Assists the Therapist in developing, implementing, and revising individual treatment plans; assures that services provided are specified in the treatment plan and monitors progress toward treatment goals.
  • Travels up to 75% within the community to patient homes or schools.
  • For adults, provides individual and group skills training and development to assist the patient and/or family in developing skills (e.g., Self-care, daily life management, interpersonal communication, parenting, illness management, anger management, positive coping skills, and problem-solving skills) directed toward reducing psychosocial barriers.
  • For children, provides individual and group skills training and development to assist the patient and/or family in developing skills (e.g., Coping with body image changes, coping with hormonal-induced emotional mood swings, self-care, daily life management, interpersonal communication, illness management, anger management, positive coping skills, and problem-solving skills) directed toward achieving developmental milestones and reducing psychosocial barriers.
  • Provides case management services as identified in the patient’s treatment plan.
  • Works collaboratively with the Therapist and medical staff to ensure coordination of integrated care for the patient, including identification of health issues, compliance with treatment, and effective patient to medical provider communication.
  • Completes CANS/ANSA re-assessments.
  • Provides consultation to staff on specific cases involving coordination of care.
  • Maintains positive working relationships with staff of other agencies and institutions, homes, and facilities, and acts as a liaison between patients and agencies and/or institutions.
  • Prepares and maintains clinical records, reports, and forms and performs other administrative tasks as required.
  • Participates in continuing education activities, remaining knowledgeable in areas of expertise.
