About The Position

The Segue Residential Transition Program assists with helping individuals transition out of state psychiatric hospitals. Segue clients are clinically stable and ready to leave the hospital. The services provided assist clients with learning how to manage their illnesses and develop independent living skills.

Requirements

• High school diploma or GED
• Two years of experience
• A driver’s license with 3-points or less
• Access to an insured vehicle

• Bachelor's degree preferred

Responsibilities

• Assist the program coordinator and director in overseeing services, staff, administration, and evaluation of the program.
• Ensure the smooth day-to-day operations and support clients in recovery.
• Participate in individual service planning for clients by assessing the provision of client health and resource management.
• Support the implementation of new and existing program services and quality improvement systems that evolve with the changing needs of clients.
• Provide the support, training, and structure needed to assure budget projections are being met.
