Requirements

  • Maintenance of an acceptable CORI
  • Negative Drug Screening
  • Valid driver’s license
  • Use of personal vehicle for work purposes
  • Provide management oversight during inclement weather
  • Maintain flexibility with weekly scheduled hours to accommodate program needs
  • Demonstrated commitment to the philosophy of community inclusion
  • Three years of experience in Human Services
  • Proven leadership skills
  • High school diploma/GED
  • Proven track record as a team player capable of influencing all levels of management.

Responsibilities

  • Directly supervise all program staff during their shifts.
  • Ensure all program support and services are provided while on shift, including supporting coordinators with the program schedule and assisting the Residential Coordinator 2 with finding coverage.
  • Ensure all program staff maintain expected levels of participation in program operations.
  • Provide on-the-job training, mentoring, and modeling for all employees and direct their activities during their shifts.
  • Ensuring the home has an inviting physical appearance, reflecting the interests and preferences of the individuals supported, creating a comfortable and personalized living environment.
  • Ensure that the home complies with licensing indicators and safety regulations outlined in the Safety Plan.
  • Submit work orders for any areas that require assistance from the facilities department.
  • Assure that ISP goals and objectives are followed through with and that required documentation exists for each goal area.
  • Communicate effectively with the Residential Coordinator 2 and clinical department to ensure all needed support is available to each area of interest and/or need.
  • Evaluate the unique support needs not included within the ISP process and utilize all resources delivered to address these needs.
  • Attend all required meetings and training.
  • Track individual entitlements and expenditures using standard accounting practices.
  • Assure staff decisions are based upon the individual's choice and empowerment.
  • Ensuring all documentation is collected and submitted to the electronic database.
  • Ensure that all required documentation is completed promptly and by established guidelines.
  • Promoting high employee morale and cultivating a positive work environment.
  • Positively and accurately promotes and plays a crucial part in the organization's mission, vision, and values of community, commitment, and continuous Learning.
  • Ensuring and enhancing community integration for the individuals we support with a person-centered approach.
  • Responsible for the outcome of the Survey and Certification Review.
  • Maintaining open and effective communication with DDS Service Coordinators, parents, guardians, employees, other team members, vendors, and the community.
  • Report any suspicion of abuse or neglect to DPPC as a mandated reporter.
  • Ensure that medications are available at home and the medication ordering protocol is followed.
  • Ensure the Medication Administration Program (MAP) policy is adhered to.
  • Assist with any medical appointments and all necessary follow-ups.
  • Report all program communications promptly to the Residential Coordinator 2.
