About The Position

The Shared Living Technical Assistant supports the overall performance and quality of the Shared Living Department, with a primary focus on the Community Companion Home (CCH) program while also providing support to the Supportive Housing program as needed. This role works closely with the Shared Living Coordinator to ensure continuity of care, regulatory compliance, and person-centered service delivery across programs. The position serves as a key liaison between providers, served individuals, internal teams, and DDS. Core responsibilities include assisting with the development and implementation of individual and support plans, supporting provider licensing and training, and conducting regular (at minimum monthly) monitoring of placements to ensure quality and compliance. This role requires flexibility, travel, and the ability to collaborate across programs to maintain consistent, high-quality support for individuals in shared living settings.

Requirements

Knowledge and understanding of Intellectual Developmental Disabilities.
An associate degree or working towards a bachelor’s degree in the Human Services field.
Understanding of DDS regulations, funding guidelines, and program models including CCH and IHS.
Must have a solid command of Microsoft 365 products not limited to Excel, PowerPoint, Word, and Outlook.

Responsibilities

Monitor assigned CCH Homes to ensure compliance with DDS licensing regulations and Quality Services Reviews (QSR) indicators.
Visit CCH individuals and providers in their home minimally one time a month.
Maintain documentation for every visit and contact of the individual and the provider.
Attend and report at all Individual Planning meetings including quarterly meetings.
Assures individual finances are used appropriately and accurate records are maintained.
Assures individual medication and medical needs are addressed and documented accurately and in a timely manner.
Ensure families are provided with all necessary material and resources to become a qualified provider.
Ensure a safe and clean environment in the home and that proper supplies are in the residence.
Assist in ensuring the oversight of Supportive Housing with staffing and documentation of services.
Collaborate with team members to ensure consistency of standards and practices across CCH and Supportive Housing program.
Assists CCH licensees in preparing for DDS Licensing inspections.
Assists CCH licensees in completing plans of correction.
Ensure and support compliance with all DDS licensing regulations from conception and throughout the partnership including home visits, initial interviews, check of physical and fire safety requirements, specialized training programs, annual background checks, state license checks, reference checks, initial home study profile, and a social summary of the household, etc.
Adhere to all DDS, state/federal and agency policies and procedures including but not limited to staff training, participant programs, medical and financial files, and fire safety/emergency requirements.
Develop and monitor CCH Individuals and Licensees, includes establishing policies and procedures that meet all DDS requirements while effectively supporting our families, team members, and Individuals, etc.
Train and supervise licensees and their designees.
Serve as a liaison between licensee, natural family, and others in relations to policies, procedures, individual care, and other concerns.
Prepare necessary reports, evaluate the progress of the individual, recommend future goals and objectives and prepare outcome measure to evaluate progress.
Work with clinical/nursing and full support team to identify needs and coordinate support efforts.
Document house specific and consumer-related information in accordance with state and agency regulations.
Verify doctor appointments and track accordingly and oversee individual's financials and records.
Participate in the Individual Planning meetings and serve as the prime spokesperson for any support needs and ensure licensee has an understanding and input in the Individual Planning Process and that that individual’s outcomes are carried forth in a timely and correct manner.
Assist in developing and monitoring weekly activities to ensure community involvement.
Keep informed and implement current trends and methods of working with people with developmental disabilities.
Participate in the recruitment of families, Individuals, and resources that meet each program needs.
Assist in the creation and deployment of marketing materials that drive interest in our programs and help recruit/obtain families and resources. This includes leveraging multiple marketing platforms including digital, print, public speaking engagements, trade shows/career fairs, mailings to private agencies, civic groups, etc.
Participate in cultivating an active network of licensee’s and individuals with interest in our programs.
Coordinate activities that support this objective including meeting with other agencies/providers, DDS Case Managers, and brokers.
Assist in matching appropriate individuals to available programs through a strenuous process of assuring commonalities and developing a network of resources for referrals.
Communicate regularly to the management team and others on program performance, successes, and issues.
Partner with other leaders and administrative team members to share expertise and insights.
Identify and participate in projects/initiatives that contribute to long-term excellence and individuals’ growth.
Work with DDS resources and Case Managers to gain expertise and knowledge.
Develop best practices for reference.
Maintain certifications, licenses, education, and training as deemed necessary by the agency.
Develop an understanding of the workings of each department and team in the company.
Perform special projects/assignments as needed.
Performs on-call duties as assigned, responding to on-site emergencies as needed.
Must be able to work after business hours and on weekends to accommodate the needs of the Provider, Individuals and the responsibilities of the on call.
Complete electronic verification of services for billing documentation for the Shared Living programs.
