About The Position

This position coordinates and monitors services for individuals with disabilities in Shared Living (host/companion) settings. The Shared Living Coordinator serves as a liaison between the organization, shared living providers (independent contractors), individuals receiving services, families/guardians, and funders. This position monitors service quality and ensures compliance with organizational, contractual, funder, and accreditation requirements. The Shared Living Coordinator provides communication and support to independent contractors regarding program, regulatory, and contractual requirements without directing the means or methods of service deliver, in order to preserve the independent contractor relationship and maintain provider autonomy.

Requirements

  • Bachelor’s degree in human services or related field, 1 year of experience working with individuals with disabilities. Or, without degree, 2 years of experience working with individuals with disabilities.
  • Must have knowledge of Microsoft Office suite including Outlook, Word and Excel.
  • This position requires driving personal vehicle in service area
  • Must have reliable vehicle and maintain current driver’s license and proof of insurance.
  • Must complete and maintain all required trainings/certifications.
  • Must maintain confidential information in accordance with HIPAA regulations.

Responsibilities

  • Reviews Person Centered Support Plans (PCSPs) and monitors service implementation to confirm alignment with plan requirements, regulatory expectations, and contract terms. Ensures regulatory and contractual requirements are communicated to independent contractors without prescribing service delivery methods.
  • Develops and maintains tracking systems to support compliance monitoring, quality assurance, and evaluation of service outcomes.
  • Conducts reviews to assess quality, outcome, and program compliance. Analyzes trends, including medication errors, reportable events, and other indicators. Notifies contractors of applicable regulatory or contractual requirements, while ensure contractors retain full discretion in determining how to address requirements.
  • Meets with individuals, families, and independent contractors to review progress toward plan outcomes, gather feedback, and facilitate communication. Clarifies applicable regulatory and contractual requirements as needed. Supports resolution of concerns while maintaining independent contractor autonomy over service delivery.
  • Reviews and maintains required documentation and individual records to verify compliance with agency, funder, and accreditation standards. Conducts audits of individuals’ financial accounts as required. Prepares and submits reports within designated timeframes.
  • Coordinates the shared living provider vetting process; conducts structured reviews focused on service quality; and matches individuals with appropriate independent contractors based on plan requirements, individual preferences, and compatibility factors.
  • Communicates contract renewal timelines to shared living providers (independent contractors).
  • Participates in meetings, trainings, and professional development to maintain knowledge of regulations, best practices, and organizational procedures.
