The Service Facilitator is responsible for coordinating and monitoring Medicaid Home and Community-Based Waiver services to help individuals safely remain in their homes and communities. This is part time with a caseload of 30 - 40 clients. This position conducts assessments, develops and updates person-centered Plans of Care, and completes required routine and reassessment visits in compliance with Virginia DMAS regulations. The Service Facilitator monitors each individual's health, safety, and welfare while identifying changes that may require additional services or interventions. The role requires maintaining accurate documentation, submitting required reports on time, and collaborating with members, families, healthcare providers, and community resources. Strong organizational, communication, and time-management skills are essential, as this position requires working independently and managing a flexible schedule. Reliable transportation and the ability to travel throughout Northern Virginia are required.
