About The Position

Moms In Motion is seeking compassionate, organized, and community-focused individuals to join their team as Service Facilitators (SF) Case Managers. In this role, you will connect with families, help them navigate Virginia’s Medicaid Waiver programs (CCC+, CL & FIS, EPSDT), and ensure those you serve can live safe, healthy, and independent lives. This position involves daily local travel to client homes and remote work from a laptop or tablet.

Requirements

Must live in VA.
At least 2 years of experience supporting individuals with disabilities or the elderly.
No degree required.
Tech-savvy enough for email, docs, spreadsheets, portal navigation and video calls.
A valid driver’s license & reliable vehicle (no client transport).
Great communication—both written and spoken.
Able to pass a background check + provide 2 professional references.

Visit clients in their homes locally.
Guide families through waiver programs.
Write plans of care and assessments.
Troubleshoot challenges such as service authorizations and timesheets.
Build relationships with families.
