About The Position

Under the general direction of a Case Management Supervisor, the Service Coordinator is responsible for the full range of case management duties, including cases requiring intensive case management and high levels of expertise. This role implements clients’ service plans. If assigned to support young children (ages 3-6), the Service Coordinator will assist Intake with determining Lanterman eligibility for children under “Provisional Eligibility” criteria prior to turning 5 years old. The Service Coordinator manages the delivery of services to regional center clients, including those with intensive physical or emotional needs, ensuring that the client’s Individual Program Plan and its objectives are pursued. They assist clients in using public and private resources before utilizing regional center funds to purchase services, monitor vendor services, and verify that services are delivered according to purchase of service authorizations. The Service Coordinator evaluates the effectiveness of clients’ Individual Program Plans, ensures appropriate community resources are made available to the client, and conducts annual reassessments of clients’ developmental status with the interdisciplinary team. They may conduct client assessments occasionally and, depending on caseload assignments, may be involved in Lanterman eligibility determination for children under “Provisional Eligibility.” The Service Coordinator may also act in a supervisory or lead capacity, such as Officer of the Day or Acting Supervisor, as assigned.

Requirements

  • A Master’s degree in a field related to developmental disabilities.
  • OR Bachelor’s degree in a field which offers content related to developmental disabilities, such as social work, psychology, counseling, a special education or nursing and three years employment in a field related to developmental disabilities.
  • Ability to provide personal automobile transportation during working hours and after hours emergencies or access to another form of reliable transportation which meets these minimum legal requirements.
  • Candidates must live within commute distance to their preferred NBRC office, as well as the geographical location of their assigned clients.

Nice To Haves

  • Bilingual Spanish/English skills are preferred.
  • Fluency in a language in addition to English is desirable and may be required.
  • Bilingual: English/Spanish preferred.

Responsibilities

  • Assist Intake with determining Lanterman eligibility for children under “Provisional Eligibility” criteria prior to turning 5 years old (if assigned to support young children ages 3-6).
  • Act occasionally, as assigned, in a supervisory or lead capacity, e.g., Officer of the Day or Acting Supervisor.
