About The Position

The Key Program, Inc. is seeking a dedicated Service Coordinator (Case Manager) to support youth and families served through the Lead Agency Program in the Greater Springfield Area. This role is embedded directly within the Department of Children and Families (DCF) regional office and plays a vital part in coordinating services, supporting placement decisions, and ensuring high‑quality care across providers. The Key Program is a private, non-profit human services agency whose mission is to support children, families, and emerging adults through trauma-informed, culturally grounded services that strengthen resilience, foster belonging, deepen community connections, and build a foundation for lasting well-being. Key works in conjunction with the MA DCF, RI DCYF and NH DCYF and is an agency provider of community-based services, residential treatment programming, and outpatient mental/behavioral health services. Requirements

  • Bachelor’s degree in a related human service or clinical field OR Minimum of 1 year case management or direct care experience
  • Must be 21 years of age or older
  • Valid state driver’s license and legally registered and insured car for work use

Responsibilities

  • Coordinating and managing services between families, DCF, and provider agencies to ensure appropriate placement and support
  • Provide referral information to potential service providers, schedule utilization reviews and ensure that appropriate services are provided to DCF children and families
  • Facilitating treatment plan reviews, diagnostic conferences, and discharge planning meetings
  • Monitoring service implementation and ensuring consistent case coordination
  • Collaborating with providers to ensure accurate and timely documentation, billing, and data entry
