About The Position

CommonBond Communities is seeking a Service Coordinator - Tier 2 for the Marketplace Crossing location. This role involves providing independent service coordination for residents, including those in permanent supportive housing, and promoting community building and engagement. The position emphasizes motivational interviewing, person-centered planning, compliance adherence, and budget stewardship, while also contributing subject-matter expertise to the team. The Service Coordinator II will be responsible for comprehensive person-centered coordination services, including conducting strengths-based assessments, developing resident-centered service plans, responding to resident crises, and managing a caseload of permanent supportive housing households. This role also involves implementing an integrated housing and services model with 3rd party property management partners, collaborating on lease violations, and coordinating case conferences for complex tenancy issues. Additionally, the Service Coordinator II will inform residents about community information and programming, gather resident feedback, involve residents in activities, collaborate on program design, and empower residents to lead on issues they care about. The position operates with moderate autonomy, independently prioritizing caseloads, escalating risk, and adhering to program standards, while influencing site-level practices and serving as a resource to peers. The role requires maintaining timely, accurate, and secure resident records in ETO, upholding program compliance, and following organizational policies for expense management and budget tracking. The Service Coordinator II is expected to contribute to continuous improvement of documentation practices and data quality, maintain communication flow, and function as a team leader by following through on responsibilities, engaging in constructive problem-solving, providing necessary information to team members, and giving and receiving feedback. Utilization of Microsoft 365 tools, including Teams and Outlook, is expected for written communication.

Requirements

  • Minimum 2 years demonstrated experience working within diverse communities, which include people of various ages, gender, sexual orientations, national origins, income levels, ability levels, religious affiliations, races, and languages.
  • Knowledge of community resources and service networks.
  • Excellent interpersonal, verbal, and written communication skills.
  • Ability to work well in a collaborative team environment with both internal and external partners.
  • Ability to network and develop community partnerships
  • Experience using MS Office 365 and other software programs.
  • Be certified in CPR and First Aid or can become certified within six months of starting position.

Nice To Haves

  • Bachelor's degree in social work or related field OR at 2 - 4 years of work experience in a related field.
  • Licensed mental health professional or social worker.
  • Experience working with individuals and families transitioning out of homeless and working with coordinated entry system.
  • Knowledge of affordable housing programs.
  • Multilingual.

Responsibilities

  • Provides comprehensive person-centered coordination services for residents
  • Conduct thorough strengths-based assessments; develop and drive resident-centered, asset-based service plans using motivational interviewing.
  • Respond to resident crises independently, with consultation as needed; coordinate warm handoffs to clinical or emergency services.
  • Manage a defined caseload of permanent supportive housing households with proactive outreach and engagement, when assigned.
  • Implement integrated housing and services model with 3 party property management partners
  • Collaborate with 3rd-party property management to address lease violations and coach residents back to good standing.
  • Coordinate multi-party case conferences for complex tenancy issues.

  • Inform residents of home base site(s) of important information and programming opportunities happening at their housing community through a monthly newsletter.
  • Listen intentionally to residents, gathering ideas on programming and services they want to see.
  • Involve residents in activities and programs taking place within their housing community.
  • Collaborate with residents on the design of activities and programs taking place within their housing community.
  • Empower residents to lead on issues they care about within their community.
  • Works as an effective team member
