About The Position

Alta California Regional Center (ACRC) is seeking a Service Coordinator in the Intensive Case Management Unit in their Sacramento office. This position will provide case management services and advocacy for persons with developmental disabilities who are conserved by the Department of Developmental Services (DDS). DDS-conserved clients may reside in their own home or with family or in a licensed, residential setting. The successful candidate for this position will provide quarterly face-to-face monitoring visits and fulfill monthly reporting requirements. Typical duties include assisting clients and their families in acquiring and maintaining assessed supports and services including transportation, vocational training, day programming, education, mental health services, medical care, residential placement if needed, and independent living training; serving as an advocate for the client with community agencies; consulting with and assisting vendors with certifications, applications, and referrals; participation in the development of Individual Program Plans for clients; monitoring these plans and revising them as necessary; corresponding and collaborating with the DDS conservatorship liaison as needed; completing all required forms, documentation, and reports in accordance with law, regulations, and ACRC policies and procedures; rotating officer-of-the-day duties with other Service Coordinators in the unit; and assisting co-workers with special projects.

Requirements

  • Bachelor’s or Master’s degree in Social Work or a related field.
  • Excellent written, verbal, and interpersonal communications skills.
  • Strong time management and organization skills.

Nice To Haves

  • One year of professional experience in social work or a related field (preferably in the field of developmental disabilities).

Responsibilities

  • Provide case management services and advocacy for persons with developmental disabilities who are conserved by the Department of Developmental Services (DDS).

  • Rotate officer-of-the-day duties with other Service Coordinators in the unit.
  • Assist co-workers with special projects.
