About The Position

The Beverly Hills CLASP (Changing Lives and Sharing Places) program is a street outreach program to engage with individuals who are experiencing chronic homelessness and mental health conditions in the city of Beverly Hills. The Service Coordinator I (SCI) will respond to outreach calls to engage unhoused individuals and provide them with necessary referrals to mental health, Veterans, and housing programs. Step Up embraces Wellness, Rehabilitation and Recovery, Housing First and Motivational Interviewing and promotes the use of these principles and practices daily when interacting with clients.

Requirements

  • Ability to engage homeless individuals on the streets in a meaningful and nonthreatening manner, evaluate and assess their needs, and begin to develop trust enough that they can accept supports and services offered.
  • Knowledge of or experience with DSM V diagnoses and level of functioning.
  • Knowledge of Evidence-Based Practices (EBP), including Motivational Interviewing, Trauma-Informed Care, Housing First, and Harm Reduction.
  • Should have the ability to work independently and collaboratively with team members, take initiative, and embrace a solution focused practice approach.
  • BA in psychology or social work, or 2 years of related work experience.
  • A minimum of 2 years’ experience working with persons experiencing mental health disorders and homelessness, in a community setting.
  • Case management experience is essential.
  • Candidate must have own transportation and present evidence of current California drivers’ license and insurance.
  • Position requires frequent driving.

Responsibilities

  • Work as part of a multi-disciplinary team in coordination with the City of Beverly Hills while serving adults on the streets of Beverly Hills and the local surrounding community.
  • Collaborated with a licensed staff to further evaluate for a qualifying mental illness diagnosis. This will include level of functioning, support network, adequacy of living arrangements, financial status, physical health, level of self-care, medication and treatment history and compliance.
  • Evaluate for immediate needs such as crisis intervention, medical attention, showers, clothing, food, emergency shelter, etc. Make referrals as needed.
  • Develop rapport and continue to make street outreach to same individuals while continuing to promote linkage to services.
  • Assist with benefits establishment, such as General Relief, Medi-Cal, Social Security, etc.
  • Administer and input required information to the Homeless Management Information System (HMIS).
  • Input and maintain clear and thorough outreach notes for each encounter into the Apricot electronic health record.
  • Attend staff meeting, supervision, and trainings as requested.
  • Other duties as assigned.
