Service Coordinator - Children's (Spanish Speakers Preferred)
About The Position
Alta California Regional Center (ACRC) is a non-profit agency seeking a Service Coordinator to provide case management and advocacy for children with developmental disabilities, ages 6-17. This position is located in the Roseville office and may involve cases throughout the Roseville area. Responsibilities include developing and monitoring Individual Program Plans (IPP), assisting clients and families in obtaining services, advocating for clients with community agencies, consulting with vendors, completing documentation, rotating officer-of-the-day duties, assisting coworkers, and providing emergency on-call services when required.
Requirements
- Master’s or a Bachelor’s degree in Social Work or a related field.
- Excellent written, verbal, and interpersonal communications skills.
- Strong time management and organization skills.
- Valid California driver license and automobile liability insurance.
- One year of professional experience in social work or a related field (preferably in the field of developmental disabilities).

Responsibilities
- Provide case management and advocacy for persons with developmental disabilities.
- Participate in the development of the Individual Program Plan (IPP).
- Monitor the IPP and assist with revisions as necessary.
- Assist clients and their families in acquiring and maintaining assessed supports and services.
- Serve as an advocate for the client with community agencies.
- Consult with vendors, applications and referrals.
- Complete all required forms, documentation and reports.
- Rotate officer-of-the-day duties with other staff.
- Assist co-workers with special projects, unique problems, vacation and sick relief as required.
- Provide emergency on-call services to assigned clients when required.
