The Service Coordinator II will be responsible for providing independent service coordination for residents, including those in permanent supportive housing when assigned, as well as promoting community building and engagement. This role emphasizes motivational interviewing, person-centered planning, compliance adherence, budget stewardship, and contributes subject-matter expertise to the team.
