About The Position

Accountable for revolutionizing the way we understand, predict, and consistently deliver on the needs of patients and the population – by building a pervasive, AI-powered orchestration engine that consistently drives access to the right care, the right way, and every time. Responsible for providing enterprise-wide strategic and operational leadership for Advocate Health’s integrated care coordination, navigation and patient flow ecosystem through: Building core capabilities, Optimizing resources, Scaling solutions, Accelerating impact, Monetizing platforms, Driving superior outcomes (agreed upon KPIs). Responsible for optimizing inpatient and ambulatory care management, care transitions, care navigation and coordination, access, and patient flow across the continuum, creating platform, processes and structure that ensure inpatient, ambulatory, and virtual care operations function as a single, integrated system across the enterprise. To achieve key system strategic priorities and platform solution builds the role must partner closely with physicians, nursing, IT, Virtual Health, Clinical Transformation, operations, strategy, and divisional leadership throughout the health system to translate enterprise strategy and design into coordinated execution at scale and in a way that is financial sustainable. Specifically, rewiring, designing, developing and implementing the end –to-end orchestration of Care Management, and the PACT (Patient Access & Care Team), with accountability for clinical outcomes, access, throughput, patient experience, and total cost of care. This position is accountable at the enterprise level, setting up system standards, driving performance through data and technology enablement, and leading large, complex teams across multiple geographies and care settings.

Requirements

  • Master’s degree in healthcare administration, Business Administration, Nursing, Medicine, or related field (or equivalent experience).
  • Minimum 15+ years of progressive leadership experience in healthcare, including enterprise or multimarket scope.
  • Demonstrated success leading large, complex clinical or operational organizations across multiple care settings.
  • Deep expertise in care management, patient flow, access, and clinical operations.
  • Proven ability to operate at the executive strategy level while driving disciplined execution.

Responsibilities

  • Serve as an operational architect, shaping access, efficiency and continuum optimization in designing solutions and as the primary point of contact for the vetting of Mission Control primary and secondary impacts for all technology, algorithms, care pathways, etc. that would impact patient care access and/or care coordination.
  • Serve as the enterprise “mission control” leader for care coordination, patient flow, and access across the care continuum.
  • Translate clinical enterprise and system strategy into integrated operating models, priorities, and performance expectations for all “mission control” functions and capabilities.
  • Establish enterprise-wide governance, decision rights, escalation pathways, and standard work for patient progression and care coordination.
  • Provide executive leadership and strategic direction for Inpatient Care Management, Ambulatory Care Management and Care Coordination, Utilization Management and Care Transitions (as applicable within enterprise scope).
  • Ensure consistent clinical standards, workflows, and performance metrics while enabling local flexibility where appropriate.
  • Drive integration across care settings to reduce fragmentation, improve outcomes, and optimize length of stay, readmissions, and post‑acute transitions.
  • Provide enterprise oversight of the PACT Call Center, aligning access, triage, and navigation functions with clinical care management strategies.
  • Ensure the call center operates as a clinical front door and command center, tightly integrated with ambulatory teams (and inpatient where appropriate).
  • Leverage digital tools, analytics, and workforce models to improve access, responsiveness, and patient experience.
  • Responsible for clinical quality and outcomes in the areas where clinical care is delivered as part of the “mission control” platform.
  • Accountable for deploying highly reliable people, processes and technology that drive an integrated and high performing patient experience for every encounter.
  • Responsible for optimizing operational efficiencies and performance of areas within scope of responsibility.
  • Responsible for creating and operationalizing financially sustainable models of care.
  • Uses advanced analytics and real-time data to anticipate demand, manage capacity, and drive proactive interventions.
  • Partner with key corporate partners and stakeholders to ensure transparency and accountability for outcomes and act as a key enterprise partner to physician, nursing, and clinical executives, ensuring alignment between care delivery models and operational execution.
  • Collaborate with market, medical group, and hospital leadership to deploy enterprise models while respecting local operational realities.
  • Influence system-wide change through matrixed leadership, not direct authority alone.
  • Sets expectations, identify dependencies, duplicative efforts and handoffs, clarify ownership and prioritize work.
  • Lead and develop senior leaders across multiple portfolios and geographies.
  • Build a high-performing, accountable leadership team grounded in Advocate Health values.
  • Serve as an executive role model for collaboration, urgency, and data-driven decision making.
