About The Position

The Senior Team Counselor - ACCS position at Riverside Community Care focuses on providing clinical interventions and peer/family support within ACCS Integrated Teams. The goal is to facilitate engagement, support functioning, stabilize symptoms, and promote self-management for individuals. This role assists individuals in developing skills and accessing resources for successful community living. The Senior Integrated Team Counselor works as part of a Person's Team, primarily delivering interventions outlined in the treatment plan under the clinical guidance of Integrated Team clinical staff. This position has a specialty role in at least one area, such as building community resource relationships, researching and connecting with benefits, employment/education support, or representative payee services. The role also involves providing input to person-centered assessments, service planning, and advocacy, coordination, and linkage with other services and the community. Additionally, the Senior Team Counselor models and teaches skills for relationship development and conflict negotiation, assists individuals in understanding and managing medications, and maintains clinical records according to established policies and timelines.

Requirements

  • Ability to work as a member of a team and to function independently required.
  • Valid driver’s license and dependable transportation required.
  • High School Diploma and three years related experience required.

Responsibilities

  • Provide input to person-centered assessment and service planning, and advocacy, coordination, and linkage with other services and the community.
  • Model and teach skills for development of positive relationships, and negotiation of conflict.
  • Assist persons served in understanding, managing, and dispensing medications consistent with MAP and agency policies and procedures.
  • Maintain regular daily, monthly, and annual clinical records in keeping with DMH, agency, and division policies and procedures, and within required timelines.
  • Deliver interventions identified in the treatment plan with clinical guidance from the Integrated Team clinical staff.
  • Have a specialty role in at least one area of expertise which may include: building new relationships with community resources, research and connection with various benefits and resources, employment and education, or representative payee.
