About The Position

Anoka County Community Social Services and Behavioral Health (CSS/BH) currently has one full-time Senior Social Worker position available. This role is responsible for providing assessments and early intervention for individuals referred to the Behavioral Health Intake unit. The unit serves adults by offering early intervention and resource navigation services. Using a person-centered approach, the primary goal is to meet individuals where they are at. Senior Social workers are responsible for coordinating access to appropriate county and community services tailored to mental health, substance abuse and social service needs for clients. After the assessment period is complete, clients are transferred to ongoing service teams for continued support if appropriate. Behavioral Health Intake services are open to all Anoka County residents, regardless of age, income, diagnosis, or insurance status. Referrals are accepted from a wide range of sources, including hospitals, treatment facilities, outpatient clinics, and law enforcement agencies. Requirements

  • Bachelor’s degree in Social Work or a closely related field.
  • Four years (2,080 annual hours) of job-related experience working directly with adults with SPMI or mental health and chemical health diagnoses.
  • In lieu of the required education, an equivalent combination of education and experience may be substituted on a year-for-year basis.
  • Valid driver’s license and vehicle available for business use.

Nice To Haves

  • Master’s degree in Social Work or a closely related field.
  • Current mental health professional licensure (LMFT, LPCC, LICSW, LP).
  • Current licensure as a Licensed Alcohol and Drug Counselor (LADC).
  • Five years or more (2,080 annual hours) of direct work experience working directly with adults with SPMI, mental health and chemical diagnoses.
  • Experience working within SSIS database.
  • Strong verbal and written communication skills.
  • Skilled in problem-solving, critical thinking and effective time management.

Responsibilities

  • Screen, assess, plan, refer, and provide early intervention services to adults referred to the Behavioral Health Intake unit from the county and community.
  • Conduct outreach and evaluate services designed to meet individualized mental and chemical health needs.
  • Conduct face-to-face visits with individuals in their home, in the community, or appropriate milieu.
  • Complete functional assessments, individual community support plans, and crisis plans with individuals that outline the services they are eligible to receive.
  • Help adults access county programs and community-based services for mental and chemical health needs.
  • Facilitate connection-to services such as therapy, IOP, partial hospitalization, substance abuse treatment, crisis stabilization services, and medication management.
  • Assist adults with obtaining least restrictive mental and chemical health care in the community or make referrals to higher level of care as needed.
  • Collaborate with other departments within Anoka County and the community to facilitate referrals/transfers.
  • Use SSIS and CaseWorks for timely completion of needed paperwork, case notes, and assessment documents.
  • Participate in individual and group clinical supervision and meetings as per department requirements.
  • Attend and participate in continuing education, training, or in service offerings required to maintain license in good standing.
