About The Position

The Senior Re-Housing Case Manager is a full-time, community-based position providing shelter support through comprehensive assessment of participant needs, case planning, and re-housing support to assist individuals experiencing homelessness return to stable housing as quickly as possible. The Senior Re-Housing Case Manager acts as a Team Leader and provides additional support to the Re-Housing Program Manager with administrative duties and general oversight. This position requires 2-5 years of Case Management experience and/or a Bachelor’s degree in Social Work or a related field. This position works out of Quincy. The hours are 10:00am-6:00pm, Monday through Friday, with some variation as necessary. CPR certification is required and provided by FBMS. AED and First Aid may also be required and will be provided by FBMS.

Requirements

  • 2-5 years of Case Management experience and/or a Bachelor’s degree in Social Work or a related field.
  • CPR certification required.
  • Valid driver’s license and good driving record required.

Nice To Haves

  • AED and First Aid certification may also be required and will be provided by FBMS.

Responsibilities

  • Model a full understanding of all agency systems and processes as related to Rapid Rehousing Programs.
  • Refer to and collaborate with community partners to provide well-rounded and approach to providing long- term, stable housing in a Housing First model.
  • In partnership with management and Senior Leadership, participate in strategic planning and program development. Participate in the development and revision of program specific policies; assist with the implementation of new and/or revised policies and protocols.
  • Conduct meetings with participants to develop Re-Housing Plans designed to accomplish an appropriate shelter exit strategy.
  • Provide support, crisis intervention, mediation and service coordination.
  • Maintain privacy of resident/guest information in accordance with agency policies, procedures and practices and as required by law.
  • Maintain and model professional boundaries.
  • Attend and participate in staff and collaborative meetings with partner agencies and trainings as required.
  • Assist in the training and development of new staff.
  • Complete intake process within required time frame. Enter all data points and case notes for guests in a timely and accurate manner.
  • Assist in transportation for Housing-related activities when appropriate.
  • Support participants in their adherence to program rules and regulations.
  • Collect and maintain data-based statistical information, as well as set up and maintain accurate case records.
  • Provide outreach and assessment in the community as deemed necessary by supervisor or other administrator within the organization.
  • Adhere to requirements as determined by program funders.
  • Support the Re-Housing Program Manager in administrative tasks such as report reconciliation and attendance records.
  • Any tasks as deemed appropriate by the Program Manager related to the mission of said program.
