About The Position

The Housing Supportive Services Program (HSSP) is a specialized program that provides comprehensive and intensive field-based community mental health services designed to meet the unique needs of individuals in Permanent Supportive Housing (PSH) who are formerly homeless and have a Severe Mental Illness (SMI) or Serious Emotional Disturbance (SED). As part of a multi-disciplinary team, the Senior Intake Clinician will conduct psychotherapy with adult members that have severe and persistent mental illness in the office, in member’s homes, or the field. The Senior Intake Clinician will conduct intakes and assessments at multiple project-based sites, coordinate client care and transfer to primary Clinician, when appropriate, with support from the Assistant Director of HSSP Admin. The Senior Intake Clinician’s primary responsibilities are to engage members and to coordinate mental health and recovery services that will ensure housing stability and community integration. This position is responsible for maintaining the highest quality provision of direct individual and group therapeutic services and case management services, while fostering teamwork among staff and across departments and programs. The goal of this position is to coordinate and complete the Intake and Assessment processes for HSSP Program Enrollment to ensure access to care.

Requirements

  • Master’s degree in Social Work, Psychology, or a related field of study from an accredited university.
  • Valid registration and licensure with the California Board of Behavioral Sciences.
  • Outstanding clinical review skills
  • Must have at least 1000 clinical hours accrued towards licensure.
  • Current, valid California Driver’s with an acceptable driving record and reliable vehicle.
  • Able to transport members in your vehicle.
  • Experience conducting DMH Intakes and Adult Full Assessments.
  • Experience and skills set with assessing and developing clinical diagnostic impression.
  • Outstanding clinical and strong team-building skills.
  • Detail-oriented with excellent time management, organizational, written, verbal, and computer skills.
  • Able to behave with sensitivity, tact, politeness and responsiveness with staff and clients

Responsibilities

  • Provide, coordinate, and implement mental health and recovery support services with individual members.
  • Conduct individual, family and group psychotherapy.
  • Conduct Intakes & Assessments for referred clients and coordinate with assigned provider on client disposition including diagnostic impression and coordination of psychiatry, mental health, and case management service needs.
  • Collaborate with each member to develop their individual service plan; review and update it upon completion of goals
  • Collaborate with HSSP Leadership and HSSP Teams for client care coordination during and after HSSP Intake and Assessment completion.
  • Provide crisis intervention and short-term therapy as needed to clients during the Intake and Assessment process.
  • Maintain 65% productivity expectation with their own client caseload.
  • Develop effective, trusting relationships with members, with a focus on facilitating independence and maintenance of improved physical and mental health.
  • Collaborate with each member to develop their individual service plan; review and update it upon completion of goals.
  • Assist management and other Clinicians with program evaluation and modification for future program improvements and growth.
  • Assist members with becoming involved in daily activities, socialization, health maintenance, sense of community, and progress in recovery reflective of The People Concern’s model which emphasizes customer choice, harm reduction, and psycho-social rehabilitation.
  • Work cooperatively and cohesively with other members of the HSSP team, including participation in weekly staff meetings, group clinical supervision, and trainings.
  • Develop and maintain a complete, accurate, and current member file with all funder-required documents.
  • Assist in crisis interventions as needed, including participating in on call rotation calendar for overnight and weekend After Hours Emergency Line.
