Senior Pharmacy Technician - Medication History
About The Position
The Medication History Specialist at Providence St Vincent Medical Center is responsible for conducting patient, family, or caregiver interviews to obtain a list of medications a patient was taking prior to admission. The Specialist may utilize other data sources to further investigate and complete an accurate medication list. The compiled medication list will be entered into the hospital's computerized medication reconciliation program and verified by a registered pharmacist. On occasion, the role may involve performing general pharmacy technician duties. Providence caregivers are valued and empowered within a culture of patient-focused, whole-person care.
Requirements
- H.S. Diploma or GED upon hire.
- Oregon Certified Oregon Pharmacy Technician upon hire.
- Oregon Pharmacy Technician License upon hire.
- 1 year Retail pharmacy experience or medication history technician experience.
Nice To Haves
- Graduate of a Pharmacy Technician program.
- PTCB certificate in Medication History.
- 1 year Inpatient hospital experience.
- 1 year Experience with pharmacy information systems (i.e., Epic, ADM applications).
Responsibilities

- Enter the compiled medication list into the hospital's computerized medication reconciliation program.
