Sabathani Community Center's Health Equity Department works to reduce health disparities and improve access to care for community members through direct service, care coordination, and community education. The department's programming is designed to meet residents where they are, with a focus on culturally responsive, trauma-informed care. Sabathani Community Center is seeking a Senior Outreach Coordinator to lead a Hennepin County-funded outreach program serving African American and African seniors (55+) in South Minneapolis, including seniors with chronic conditions who are not eligible for waivered services, along with their family caregivers and informal support networks. The Senior Outreach Coordinator conducts intakes and individual service plans, coordinates a volunteer Companion Program, facilitates a monthly Caregiver Circle, provides short-term case management and referrals, and manages program data and quarterly reporting. This role exists to honor elders, reduce isolation, and help seniors age with dignity, joy, and independence, and is well suited to someone who builds trust through consistent follow-through and sees the seniors they serve as community members with wisdom, stories, and gifts to share. The physical duties associated with this position are consistent with duties associated with work performed in an office and community setting.
