About The Position

This is a nursing position in the School Health program providing assessment of student health needs, referrals for services, and liaisons between schools, parents, and the school district. The incumbent is routinely responsible for routine travel to perform their duties. Reports directly to the Community Health Nursing Consultant (CHNC). Ensures day-to-day school health functions are done to ensure the completion of the school year plan between the Florida Department of Health in Osceola County and the School District of Osceola County (SDOC). School health functions include meeting screenings and immunization administration deadlines for public, charter, and private schools by the end of the school year. Ensures timely communication of planned health screenings and works with SDOC staff in planning and implementing screenings as appropriate. Reviews and updates health profiles on children and notifies school personnel as needed. Reviews SDOC and Florida Shot Database health records for student immunization status. Establishes and maintains effective communication and relationships with the School District to ensure quality school health services. Serves as resource person for teachers, health aides, students, and parents. Coordinates screening and surveillance activities with schools. Assists with referrals of individuals and families to appropriate local and state agencies. Contacts private/charter schools yearly regarding mandatory screening services available and required immunizations. Serves as a point of contact for the CHNC when school health team is working off campus. Ensures work phone is charged and turned on during work hours. Completes statistical reports and assists SDOC school nurses with documents as required. School Health reports can include the School Health Service Plan, Compulsory Report, Annual School Health Service Report, Kindergarten and Seventh Grade Assessment and Validation Report. Prepares and participates in School Health Audit. Performs QI surveys and assists in corresponding reports as necessary. Shares information with on-site school health staff and/or administrators as appropriate. Serves as liaison between parents and school regarding medical concerns. Assists the Immunization Department and updates 680 forms as needed throughout the year. May assist in clinic with problems related to school age children; also assists in Florida Department of Health clinics as needed due to absences, illness, event coverage, vacations, etc. Plans and participates in health promotion and disease prevention programs related to school children, school personnel, and the community. Assists with development of educational and training material as appropriate. Uses automobile as needed and maintains current Florida driver’s license. Maintains record of daily activities and work performed. Enters accurate daily activities into HMS in a timely manner. Performs other related duties as required. RESPONSIBLE FOR TIMELY SUBMISSION OF TIMESHEETS, EARS AND CERTIFICATION and attending all staff meetings, quarterly School Health conference calls and one-on-ones. Completes mandatory trainings as deemed necessary. Provides advanced notice for time requests (2 weeks) unless an emergency. Information Custodian responsibilities include maintaining information in accordance with state and federal statutes, rules, regulations and Department of Health Security Policy 3 and associated Protocols and Procedures as it relates to information Custodian, Delegation of Authority. This responsibility is to be adhered to 100% of the time.

Requirements

  • Licensed Practical Nurse licensure in the State of Florida or a multistate license
  • Valid Florida Driver's License
  • Willing to travel to other sites in Osceola County to perform duties
  • Willing to perform emergency duties before, during and/or beyond normal business hours and/or day in the event of an emergency, disaster, or threat of disaster man-made or natural
  • Willing to physically come into the office daily and perform field work
  • Authorization to work in the United States without employer sponsorship
  • Knowledge of nursing principles, practices and techniques.
  • Knowledge of human anatomy and physiology.
  • Ability to administer prescribed medications.
  • Ability to provide direct patient care and treatment.
  • Ability to work with patients having a variety of physical problems.
  • Ability to plan, organize and coordinate work assignments.
  • Ability to communicate effectively.
  • Ability to establish and maintain effective working relationships with others.
  • Ability to learn and communicate effectively, orally and in writing, in English.

Nice To Haves

  • At least one (1) recent year of experience as a Licensed Practical Nurse
  • At least one (1) or more years of experience in any of the following: School Health, Primary Care, hospital setting, or outpatient clinical settings
  • Experience in documenting and retrieving clinical information on the computer
  • Experience with Microsoft Office (Word, Excel, PowerPoint, Outlook)
  • Bilingual in English and Spanish and fluently speak, read, and write both languages
  • At least one year experience providing immunizations
  • Experience documenting in the FL Shots registry
