About The Position

BAYADA Home Health Care has an immediate opening for a Senior Habilitation Manager in our Philadelphia, PA office. This role is for an experienced professional passionate about supporting individuals with intellectual and developmental disabilities (I/DD), seeking to advance their career and make a meaningful impact. As a Senior Habilitation Manager, you will be a key advocate and resource for clients, providing leadership and oversight to Habilitation Technicians. You will collaborate closely with clients, families, Case Managers, Care Coordinators, and interdisciplinary teams to ensure services align with individual goals and needs.

Requirements

• Bachelor's degree required
• Minimum of 2 years of post-degree experience working with individuals with intellectual and developmental disabilities (I/DD) required
• Strong understanding of person-centered planning and individualized support practices
• Demonstrated success in taking on increasing levels of responsibility and achieving goals
• Strong communication, organizational, and interpersonal skills
• Ability to collaborate effectively with clients, families, and multidisciplinary teams
• Passion for supporting individuals with I/DD and promoting independence, dignity, and quality of life

Nice To Haves

• Prior supervisory or leadership experience preferred

Responsibilities

• Serve as an internal case manager and advocate for clients receiving Habilitation services
• Participate in Person-Centered Planning and Individual Support Plan (ISP) processes
• Collaborate with clients, families, Case Managers, Habilitation Technicians, and other care team members to identify, implement, and evaluate individualized goals and service needs
• Conduct regular client visits to assess progress and ensure services are delivered in accordance with the Person-Centered/Individual Support Plan and applicable service definitions
• Evaluate client progress toward established goals and identify opportunities for continued development
• Maintain consistent communication and collaboration with Case Managers and Care Coordinators
• Provide supervision, coaching, and support to Habilitation Technicians in accordance with company policies and applicable state regulations
• Participate in client-specific orientations and training as directed by the Clinical Manager
• Support team members in navigating complex client needs and service delivery challenges
• Contribute to program initiatives and quality improvement efforts
• Serve as a resource and mentor to Habilitation Managers and other team members as needed
