About The Position

In collaboration with UI Health Care hospital administrative leaders, physicians, and community partners, the Senior Director of Home Health Services provides strategic and operational leadership for the comprehensive array of services supporting patients transitioning from hospital to home. This includes oversight of homecare nursing services, home infusion services, durable medical equipment (DME), and home health medical supplies across inpatient, outpatient, and procedural settings throughout the health system and defined retail areas. The Director is responsible for setting priorities, evaluating outcomes, and advancing the home health strategy to ensure safe, timely, and coordinated discharges. This leader drives the vision, planning, and continuous improvement of service delivery, patient experience, financial oversight, accreditation adherence and operational performance, working closely with executives, clinical departments, and external providers to enhance continuity of care. This position plays a critical role in optimizing patient flow, reducing readmissions, improving patient satisfaction, and ensuring the health system’s home health operations align with regulatory standards, financial goals, and the organization’s mission to deliver exceptional care beyond the hospital walls.

Requirements

  • A Master’s degree in Health Care Administration, Business Administration, Nursing or related field, or an equivalent combination of education and experience that demonstrates an advanced knowledge of homecare, DME, infusion as well as hospital clinical systems, policies and procedures in order to direct, plan and implement various homecare systems.
  • 5-7 years of clinical leadership experience related to program specialties that demonstrates knowledge of organization, operation, and coordination of home health activities. Track record in program integration, execution, development and expansion required.
  • Excellent consultative, collaboration and communication skills necessary to partner with teams across the Health System, physicians, patients, families, and others to achieve goals.

  • Analytical abilities to develop, design, implement and evaluate systems and prepare analysis identifying critical concerns and recommending corrective action and to develop and administer policies, budgets, operational models and oversee work of others.
  • Executive presence and demonstrated interpersonal skills necessary to provide effective leadership.
  • Professional job-related experience fostering or promoting a welcoming and respectful work/academic environment.

Responsibilities

  • Serve as a liaison among hospital staff, care coordination teams, home health agencies, and leadership to ensure seamless patient transitions from inpatient to home settings
  • Partner with physicians and interdisciplinary teams to identify opportunities to streamline discharge processes and expand post-acute service offerings.
  • Develop and maintain strong relationships with executive leadership, clinical departments, and community partners to align priorities for home health services.
  • Coordinate efforts across patient care services, ambulatory care, surgical services, support services, finance, and quality/safety teams to ensure consistent and effective operations.
  • Serve as a visible and accessible leader, promoting awareness of home health services and fostering collaboration across the continuum of care.
  • Lead the development, implementation, and evaluation of strategies to enhance the effectiveness and efficiency of homegoing operations.
  • Develop business plans to evaluate current service offerings as well as identifying areas of opportunity for growth and expansion.
  • Ensure alignment of discharge planning, DME, home infusion, and homecare services with clinical practice standards, safety requirements, and patient-centered goals.
  • Partner with departments such as Marketing, IT, Nursing, Finance, Pharmacy, Supply Chain and Strategy to support service integration, data tracking, and communication strategies.
  • Use data and analytics to identify opportunities to improve patient outcomes, reduce barriers to discharge, and enhance access to home-based services. Develop and enhance KPI indicators and visual dashboards to allow for ease of understanding programmatic performance
  • Represent home health services in internal and external forums, promoting best practices and innovation in transitional and home-based care.
  • Keeps abreast of industry trends and regulatory requirements by participating in state and national level industry associations
  • Build, lead and mentor a multidisciplinary team, including program managers, coordinators, and support staff, to ensure high performance and engagement.
  • Foster a culture of teamwork, accountability, and continuous improvement across all aspects of homegoing operations.
  • Collaborate with HR and departmental leaders to support recruitment, professional development, and performance management.
  • Evaluate staffing models to ensure resources align with service volume, quality goals, and operational priorities.
  • Ensure home health services comply with all federal, state, and accreditation requirements.
  • Partner with Quality and Compliance teams to monitor performance metrics related to patient safety, readmissions, and discharge efficiency.
  • Use data-driven approaches to evaluate process performance and implement improvements in service delivery and patient satisfaction.
  • Work with the Office of the Patient Experience to evaluate, monitor and improve the impact that the home health program has on patient satisfaction.
  • Collaborate with care management and nursing leadership to implement best practices in care transitions and post-acute coordination.
  • Support the integration of technology and digital tools to streamline communication, referrals, and tracking of patient outcomes.
  • Develop, monitor, and manage budgets related to DME, supplies, and home health operations.
  • Evaluate utilization patterns, costs, and reimbursement trends to identify opportunities for operational and financial optimization.
  • Partner with Finance and Supply Chain teams to develop strategic sourcing, contracting, and cost-containment initiatives.
  • Analyze key performance indicators and adjust operations to meet budget, quality, and patient care objectives.
  • Stay informed on healthcare regulations, reimbursement models, and emerging trends affecting homegoing and post-acute services.
