About The Position

Madrigal is a biopharmaceutical company focused on delivering novel therapeutics for metabolic dysfunction-associated steatohepatitis (MASH), a serious liver disease that can progress to cirrhosis, liver failure, need for liver transplantation and premature mortality. Madrigal’s medication, Rezdiffra (resmetirom), is a once-daily, oral, liver-directed THR-β agonist designed to target key underlying causes of MASH. Rezdiffra is the first and only medication approved by both the FDA and European Commission for the treatment of MASH with moderate to advanced fibrosis (F2 to F3). An ongoing Phase 3 outcomes trial is evaluating Rezdiffra for the treatment of compensated MASH cirrhosis (F4c). Our success is driven by our people. Senior Director, National Accounts (ESI/Cigna)- MASH The Senior Director, National Accounts is responsible for leading Madrigal's engagement with key national payer and PBM accounts to optimize patient access, strengthen account performance, and support business objectives across the MASH portfolio. This role reports directly to the Executive Director, National Accounts and serves as a senior strategic partner across National Accounts, Regional Account Directors, Market Access, Finance, Analytics, Trade, Specialty Pharmacy, and Field teams. The Senior Director translates payer dynamics, contract structures, access performance, and customer insights into clear strategies and execution plans. As a senior leader, this individual will manage complex national account relationships, support contracting and financial performance objectives, and ensure payer wins translate into real-world access, utilization, and field execution.

Requirements

  • Bachelor degree required; advanced degree such as MBA, MPH, PharmD, or related discipline preferred.
  • 12+ years relevant pharmaceutical or healthcare related work experience
  • 6+ years of experience in Market Access, payer strategy, national account management, contracting, finance, analytics, or related pharma, biotech, payer, or health plan roles.
  • Demonstrated experience managing national or large regional payer accounts, including PBMs and/or large commercial plans.
  • Strong understanding of the U.S. payer landscape, formulary management, utilization management, payer contracting, rebate structures, gross-to-net, Specialty Pharmacy, reimbursement, and patient access pathways.
  • Proven ability to manage complex customer relationships, influence cross-functional stakeholders, and translate data and market insights into actionable strategies.
  • Experience supporting product launches, access strategy execution, business reviews, performance scorecards, and cross-functional reporting processes preferred.
  • Strong executive communication, analytical rigor, financial and contracting acumen, collaboration, project management, and follow-through skills.
  • Proficiency with Microsoft Office applications, including Excel and PowerPoint; familiarity with payer data, claims, CRM, specialty pharmacy, or access analytics platforms preferred.
  • Role requires up to 30% domestic travel

Responsibilities

  • Lead strategy and execution for priority national payer and PBM accounts, including development of short- and long-term account plans aligned to brand, access, and financial objectives.
  • Serve as the primary point of contact for assigned national accounts, engaging pharmacy, medical, formulary, population health, quality, and economic decision-makers.
  • Build trusted customer relationships that enable effective collaboration, negotiation, contracting discussions, and issue resolution.
  • Monitor account performance against contracted expectations, business plans, access objectives, and financial assumptions; identify risks, gaps, and opportunities for improvement.
  • Partner with the Executive Director, Pricing & Contracting, Value & Access, and Legal to support contracting strategy, rebate accuracy, contract alignment, gross-to-net performance, and financial reconciliation.
  • Analyze payer performance, utilization management, coverage quality, formulary alignment, pull-through, and operational access barriers to identify trends and systemic opportunities.
  • Partner closely with National Account Directors, Regional Account Directors, Sales, Trade, Patient Support Services to translate payer wins into field-level execution and remove barriers to patient access.
  • Develop national account performance updates, executive briefings, strategic recommendations, and customer-facing business reviews.
  • Share customer and market insights to inform messaging, tools, resource development, access strategy, and cross-functional execution plans.
  • Provide leadership, guidance, and best-practice sharing across National Accounts and cross-functional partners while demonstrating strong executive presence, accountability, and compliance.
