We provide a member-focused, pharmacist-driven care model in collaboration with health systems, assisted living facilities, and health plans. Our goal is to help our members feel better about the medications they take every day by simplifying medication regimens, improving adherence, and delivering positive outcomes. As a key leader on the Mosaic leadership team, the Director/Head of Clinical Pharmacy Services will be accountable for the design, implementation, and outcomes of Mosaic’s clinical services model. Clinical services for Mosaic members are provided by all clinicians in the Mosaic care model, which engages patients and providers to ensure each patient’s regimen is as safe, simple, effective, and affordable as possible. Opportunities for clinical intervention include initial medication reconciliation, onboarding, monthly calls for refill authorization, and comprehensive medication reviews. Clinical interventions are guided by the company’s Therapeutic Decision Support™ technology. Reporting to the General Manager with dotted line responsibility to the Chief Medical Officer, this executive will lead a highly capable team and technology platform, while also working with senior members of the Mosaic and RxAnte team to sell, deliver, and measure the performance of Mosaic clinical outcomes in collaboration with our health plan clients.

Requirements

  • Doctor of Pharmacy or equivalent experience
  • Licensed, and in good standing, to practice pharmacy in Virginia
  • 7+ years of clinical pharmacy patient care and management
  • At least 4 years of direct ambulatory patient care experience
  • Completion of an ASHP-accredited residency OR board certification (BPS) in Ambulatory Care, Geriatrics, or Pharmacotherapy OR equivalent experience supervising ambulatory clinical pharmacy care will be considered.
  • Track record of building and scaling clinical pharmacy services with proven outcomes
  • Understanding of the payer and provider markets including pharmacy quality measures, population health management, and pharmacy benefit management

Responsibilities

  • Development, implementation, and management of the Mosaic clinical pharmacy care model
  • Ensure clinical services consistently deliver superior clinical quality scores (i.e., CMS Star Ratings, HEDIS measures, etc.) and lower total cost of care
  • Drive the requirements for the procurement & development of enabling technological solutions to make the clinical model effective, efficient, and scalable
  • Compliance partner to Pharmacist-In-Charge, Legal/Regulatory to build and foster a culture of compliance
  • Maintain patient medication records according to appropriate regulatory bodies
  • Participate in the development and implementation of Quality Improvement Projects (QIPs) in collaboration with various stakeholders
  • Lead management of a team of clinical pharmacists and pharmacy technician(s). Monitor and manage team performance using tracking reports of key performance and quality indicators related to the pharmacy’s clinical model and impact on client outcomes
  • Determine staffing requirements, ensure optimal staffing levels are maintained, and develop plans to staff the clinical operation according to patient & client demand
  • Optimize resource allocation to achieve cost-effective clinical operation that meets patient and client needs, while also achieving company’s financial goals.
  • Convene and collect input from a committee of clinical and scientific leaders
  • Serve as primary supervisor and liaison for Mosaic’s preceptor program, for pharmacy school students completing their APPE rotations
  • Represent Mosaic as a presenter of research and educational content at conferences and other events
