About The Position

The incumbent is part of a global public health organization and is expected to support the Florida Department of Health in Pasco County in its mission to protect, promote and improve the health of all people in Florida through integrated state, county and community efforts and its vision to be the Healthiest State in the Nation. This role provides generalized nursing services in all public health programs, including follow-up on referrals, nursing assessments, care, and treatment in Family Health Services programs. The nurse will assess, identify, and document client and process problems, making a nursing process and diagnosis utilizing an electronic health record. Responsibilities also include providing coverage and assisting providers in clinics, case management for assigned programs, maintaining tracking systems for follow-up, and making referrals to community agencies. The position involves performing HIV counseling, referrals, and file maintenance, as well as functioning as a Triage Nurse, providing telephone triage, and administering immunizations with associated counseling and education. The Senior Community Health Nurse will also perform interviewing and assessing skills, assist physicians and ARNPs with client intake, exit, and procedures, clarify treatment plans, perform and document laboratory tests, and provide follow-up on abnormal results. Timely client management across various Family Health Services programs (e.g., Family Planning, Immunizations, STDs, HIV/AIDS) is crucial. Additional duties include assisting with inventory, maintaining Bloodborne Exposure Policy and Procedure, reviewing medical records for Quality Improvement, serving as a preceptor, orienting and training new staff, and traveling to other clinic locations as needed. The incumbent is also expected to attend professional development activities and provide nursing services during disasters.

Requirements

  • Knowledge of nursing and public health principles, practices and techniques.
  • Ability to administer medications.
  • Ability to communicate effectively.
  • Ability to teach health care.
  • Ability to utilize Electronic Health Record.
  • Licensure as a Registered Professional Nurse in accordance with Florida Statute 464.
  • Must have a valid Florida Driver’s License.
  • Must have reliable transportation.

Nice To Haves

  • 6 months of experience working as a Registered Nurse.
  • 6 months of experience using an EHR (Electronic Health Record).
  • Valid CPR/BLS Certification.
  • Proficiency in English and Spanish.

Responsibilities

  • Provide generalized nursing services in all public health programs including follow-up on referrals.
  • Provide nursing assessments, care, and treatment in all Family Health Services programs.

  • Responsible for case management of assigned programs.

  • Perform HIV counseling, make referrals, and maintain files according to policies and procedures.
  • Perform the functions of the Triage Nurse as required.
  • Perform Nursing telephone triage to the FDOH Pasco clients and/or individuals calling into the clinic Triage line.
  • Provide immunizations to adults and children as required according to the state and local guidelines and policies.
  • Provide the required counseling and education on all immunizations administered according to the state and local policies and procedures.
  • Perform interviewing and assessing skills on the health data received from all clients.
  • Assist the Physicians and Advanced Registered Nurse Practitioners (ARNP) with client intake, exit and assists with procedures.
  • Responsible for reviewing and/or clarifying the Physicians or ARNP treatment plan to clients.
  • Perform and document laboratory tests and provide follow-up on abnormal results.
  • Provide HIV pre and post counseling and testing per policy and protocol.
  • Responsible for timely client management, which includes assessment, planning, care implementation, counseling, education, referral, follow-up and documentation for Family Health Services programs including but not limited to: Family Planning - Title X, Sterilization, Teens, Immunizations Childhood and Adult, Sexually Transmitted Diseases, Tuberculosis, Hep C treatment, PreP Initiatives, HIV/AIDS etc.
  • Perform laboratory and other diagnostic tests as indicated.
  • Assist with ordering and maintaining all inventory (supplies/medication/vaccines).
  • Maintenance of Bloodborne Exposure Policy and Procedure, as well as general backup for the department.
  • Review medical records at least monthly for Quality Improvement (QI) maintenance, collate, and document information as needed for monthly report.
  • Serve as a preceptor for nursing students and trainees.
  • Orient and train new members of DOH Pasco staff.
  • Travel to other clinic locations in Pasco to provide direct client services as needed.
  • Perform other duties as required.
  • Attend workshops, meetings, and staff meetings as indicated and participate in activities for professional development.
  • Provide nursing services during a disaster, according to the FDOH Pasco's Nursing Policy and Procedure.
